Please Note:
Linen rental prices do not include candle wax,
 ink or paint removal. 

"What am I responsible for?"
You are held responsible for linens returned with  excessive amounts of food attached to them, 
 candle wax, paint, handwriting from ink pens or markers, etc., burns, mildew, rips/tears,
 and excessive "ground in" dirt.

Tablecloths returned with any of these will not be processed, taken out of our inventory,
 returned to you, and we will charge you a reasonable replacement fee.

Submit this form if:
n You have already placed a phone order with us and been given a contract number.

n You agree to pay the replacement cost for linens returned to us that are damaged,
 defaced, or in a condition deemed by us to be non usable for future rental use.

n You agree to pay the rental amount  for the quantities requested by you when you placed
your order.

 n Acknowledge that refunds or credits for reduction in quantities or cancellations are not guaranteed,
and will be made at the sole discretion of Kosins Rental Centers Inc.

n You have made payment , or will be making payment via valid charge card, personal or company
check, cash,  or money order prior to our delivery, or your pick-up of these items at our store.

 By completing and confirming the information fields at the bottom of this page,
you agree to the above terms and conditions of this addition to our standard rental agreement.
